We understand that patients being referred to a nephrologist can feel confused and overwhelmed. Patients with kidney disease typically don’t know they have a problem because they have no symptoms and generally feel well. Early stages can only be detected by blood and urine tests — and the earlier the detection, the greater the opportunity to stop or slow progression.
Setting up an appointment
Your primary care physician will provide us with a referral request. Our team processes referrals promptly to assess urgency and ensure all documents are complete. You’ll receive a call from our office to confirm your referral and help schedule your first appointment. Don’t hesitate to call us directly with any questions before your visit.
Appointment scheduling
Our front desk will contact you to confirm your consult visit and review basic information. A week before your appointment, you’ll receive a reminder call. Please let our office know of any specific results you’d like included for your first visit. If you need to reschedule, please contact us at least 24 hours before your visit.

How to prepare for your first appointment

Inform us prior to your visit if you have previous lab work or imaging studies from another facility.

Review your medication list to ensure it is up to date.

Prepare any documented charts of home blood pressure or weight.

Complete the requested patient forms available on our website.

Most importantly, write down any questions you’d like answered — our providers appreciate this.
On the day of your visit
- Try to arrive at least 15 minutes before your appointment time.
- Bring your photo identification and medical insurance information.
- Bring a method of payment — your co-pay is collected at the time of service.
- Bring your updated medication list or current medication bottles.
- Remember to bring blood pressure charts if available.
